The two faces of inflammation
Inflammation is not inherently harmful — it is your body's primary survival mechanism. When you sprain your ankle, cut your finger, or encounter a pathogen, your innate immune system launches an acute inflammatory response designed to isolate damage, neutralize threats, and initiate healing.
The acute cascade is orchestrated with precision: Pattern Recognition Receptors detect damage or pathogen patterns, which activates NF-kB — the master transcription factor of inflammation. NF-kB then upregulates pro-inflammatory cytokines (IL-1β, IL-6, TNF-α) and prostaglandins through the COX-2 pathway.

Resolution depends on specialized pro-resolving mediators — resolvins, protectins, and maresins — synthesized largely from omega-3 fatty acids. When resolution works, inflammation turns off cleanly. When it fails, the fire smolders indefinitely.
A persistent, sterile state where inflammatory signaling stays low but constant — never producing the classical heat, redness, or swelling of acute injury, yet quietly oxidizing lipids, glycating proteins, and impairing mitochondria year after year.
A 2018 landmark paper in Nature Medicine identified this chronic, low-grade inflammation — sometimes called "inflammaging" — as the common driver behind the leading causes of death worldwide: cardiovascular disease, cancer, diabetes, Alzheimer's, and autoimmune conditions.
The NF-kB switch
In acute inflammation it turns on, does its job, and turns off. In chronic inflammation, it stays stuck in the ON position — indefinitely.
